We consider your Primary Office Address to be the office where you see the greatest number of patients. If this happens to be a hospital, please use the address you would use for receiving correspondence at that hospital. You may wish to use a different address for your Honoraria Mailing Address.

Yes, that would be considered your Primary Office Address as well as your Honoraria Mailing Address.
I have several degrees - which one should I choose?
We recommend that you use the degree that reflects the highest level of medical training you have received.
Why do you want my e-mail address and Phone/Fax Numbers?
Often we will contact study participants via e-mail. While fax is the method of contact most often used to invite a physician to a study, we sometimes use e-mail as an alternative or additional method to contact those physicians who most closely match the criteria for our on-line studies.
We will only use your phone number to contact you about problems you may be having with one of our on-line studies. We will NOT give out you phone, fax, or e-mail address to any other party.
